Medulla Compression by Bilateral Aberrant Vertebral Artery With Mild Chiari Malformation

چکیده
A 48-year-old woman developed progressive right lower extremity numbness and lancinating pain of pharynx. T2-weighted MRI revealed a flow-void area that corresponded to the both aberrant vertebral arteries (persistent first intersegmental arteries) severely compressing distorting medulla oblongata tonsilar herniation consistent with mild Chiari malformation. Mild hydrocephalus was also noted thinning corpus callosum partial empty sella (figure).

Vertebral artery compression of the medulla.
BACKGROUND Intracranial arteries in the subarachnoid space may compress the brain parenchyma and cranial nerves. Most arterial compressive lesions have been attributed to dolichoectasia in the vertebral-basilar system, and prior reports have concentrated on the pressure effects of basilar artery ectasia. Much less is known about vertebral artery compression of the medulla. OBJECTIVE To descri...
متن کاملProvokable bilateral vertebral artery compression diagnosed with transcranial Doppler.
BACKGROUND Head and neck movements may cause vascular compression that produces a syndrome of vertebrobasilar insufficiency. Vertebrobasilar circulation was examined noninvasively in two patients who were able to provoke these symptoms repeatedly on demand. CASE DESCRIPTIONS Blood flow velocities in the basilar artery (case 1) and both posterior cerebral arteries (both cases) were measured co...
متن کاملA case of aberrant bilateral vertebral arteries orgin presenting with right VA dissection
Background: Knowledge of variations in the origin of vertebral artery (VA) is indispensable to vascular surgeons. Aberrant origin of vertebral artery on either side is an uncommon finding. There are unilateral and bilateral variability in VA origin. Case presentation: We present a case of vertebral artery dissection who was found to have bilateral VAs aberrant origin. The right VA took origin ...

A 55-year-old man presented with vertigo, nystagmus, and gait ataxia followed by left hemiparesis (Opalski syndrome). T2-weighted magnetic resonance imaging revealed vascular compression of the left lateral side of the medulla oblongata by the left vertebral artery. On diffusion tensor imaging, the level of fractional anisotropy (FA) in the left corticospinal fibers caudal to the pyramidal decu...
